    	(RHI, DHI) von Ultraschallkontrastmittel-Mikrobläschen im Hirnparenchym
    	sind neue Ansätze in der zerebralen Perfusionsdiagnostik mit Ultraschall.
    	
    	
    	Methoden: Acht Patienten mit akutem Mediasyndrom wurden mit beiden
    	Ultraschalltechniken untersucht (SONOS 5500, axiale dienzephale Untersuchungsebene
    	ipsilateral, 10cm Tiefe, RHI: steigende Pulsintervalle 250–4000 ms,
    	DHI: 6,67Hz, SonoVue-Infusion 1,6ml/min) Im zeitl. Abstand von weniger
    	als 4 Stunden wurde ein Stroke-MRT (PWI/DWI) als Referenzmethode
    	durchgeführt. Aus normierten Zeit-Intensitätskurven eines Rasters
    	von Messregionen (n=60) wurden aus den beiden Ultraschallbildsequenzen
    	die folgenden Parameter errechnet: DHI: Amplitude (A), Halbwertzeit:
    	(H), und Perfusionskoeffizient (P). RHI: Geschwindigkeitsäquivalent
    	? und Blutflussäquivalent (F). Nach Dichotomierung in normale vs.
    	verspätete Kontrastanflutung im MRT (>4s in PWI), führten wir eine
    	Receiver Operating Characteristics- (ROC) Analyse durch, die optimale
    	Grenzwerte für jedes Modell berechnete. Für jeden Kurvenparameter
    	wurden darüber hinaus farbcodierte Parameterbilder erstellt.
    	
    	
    	Ergebnisse: Für Hirnareale mit signifikant (Signifikanzniveau angeben!)
    	verzögerter Kontrastanflutung berechneten wir die folgenden Grenzwerte:
    	A: 9.8%, H: 98 ms, P: .042s-1, beta: 8.08*10–6, F: 4.81*10–7. Für
    	die Abgrenzung zwischen normalen und diffusionsgestörten Arealen
    	fanden sich keine signifikanten Grenzwerte. Die Fläche mit signifikant
    	reduziertem Perfusionskoeffizienten im DHI-Parameterbild korrelierte
    	mit der Fläche signifikanter Perfusionsverzögerung im PWI (R=.80,
    	P=.02).
    	
    	
    	Schlussfolgerung: Mit beiden neuen Ultraschallmethoden finden sich
    	signifikante Grenzwerte zur Abgrenzung kritisch-minderperfundierter
    	Hirnareale von normal perfundiertem Hirngewebe im akuten Mediainfarkt.
    	Weitere Studien zur Evaluation dieser neuen Ultraschalltechniken
    	erscheinen gerechtfertigt.},
